My form is disabled after being published to facebook and it says contact us.

This if my first form and it says it is disabled after being published to facebook.  If i click "contact us" on my facebook page it brings up the error.

Also I do not want the facebook button to say "contact us".  I want it to say "Order Here".

We apologize for the inconvenience caused. I have checked the form http://www.jotform.us/form/60126978500152 and found that it was wrongly disabled by the Phishing filter due to the field that ask for the Facebook username on the form.

I have corrected this and reinstated the form. Please check again on your facebook page.
