I think this is fixed. The issue is with the custom url I assigned to this form a while ago. To fix, I had to go the publish button and edit the link, deleting all of the previous custom url that was entered before.

I have tested your form and I was able to replicate mentioned issue.
After that I have cloned your form and I saw that your form is large, there is more than 200 fields. Also you have 222 conditions. Some of the conditions are reporting an error because they are created for fields that are deleted from your form. I would suggest you to delete those conditions.
However even when I deleted all conditions on my cloned form I was not able to open it.
I will escalate this issue to our developers and we will inform you via this thread once this issue is resolved.

Thank you very much. I can't remember every step, but I was able to get the form opened several times last night by clicking on the publish button in the editor. There is some peculiar issues with the url if you click the pencil to edit the url and then click the refresh button, then, copy and paste the url and the form will open when the url is copied and pasted from that box in the publish tab. But, it will only open one time and then have issues with URL again.
As far as the form being large, I am aware of that. I have been a loyal Jotform customer for over a year and created nearly 50 forms to meet the needs of our agency. i have researched the forums looking for an answer regarding a "maximum number of conditions" and could not find one.
It does make sense that the size of the form is bogging it down. But if that were the cause, I think there should be some way to notify the Jotform user that they are approaching a dangerous level of conditions that will impact the performance of the form. My education and background is in Human Resources, so if I don't get a warning telling me that I am approaching the limit on conditions, I will just keep writing, and writing, and writing them until what happened now. A form, I have spent, well..at least 100 hours creating is broke.
This product is marketed as a tool for users with little or no experience in coding. That is me, and that is what I love about the product. I think a lot of times that I run in to situations that caused me so much time over the past year because I think Jotform assumes their user's baseline technical knowledge is higher than what it truly is. Sorry for the rant, I am approaching the tail end of a project involving employee benefits and this form is the very last step, and the most important. So please, if the answer you have for me is going to result in a ton of work on my part, understand the amount of time that I have put in this thing and please explore every opportunity to make it right for me.

OK, I am getting the form to open up successfully every time. The last edit I made was insert an image with a mailto link inserted. I deleted that image and the form opens up. Any ideas why that may have happened?

My apologies for your frustration. Though you can add as much as you'd like to your form, keep in mind that web browsers themselves have limitations. When you load a couple hundred conditions and many widgets, it puts quite a bit of strain on the browser, especially on a slow computer with a slow connection.
I was personally able to open the form each time I attempted to, though there was quite a bit of load time. As for the image that was removed, would you mind sharing the link you used for the image? We can check to see if that may have been causing the issue.

From what I saw you deleted second image widget from your form, so I have tested further more your form and seems that link you put inside of Image widget is not valid.
Form by itself will make a link in back end and your link is has additional a href tag:

When I removed a href tag I decoded link because seems that encoding/decoding could be a problem for resolving mailto links and after decoding I got this link:
