I cannot format the header of my form

    I am trying to place the logo at the top of the header in the center with the text (title below).  I thought this the easiest solution and I am still challenged.  Preferably, I would like it placed to the left of the text with the title right justified.  I cannot figure out either one.  Also when I looked at the form to get the URL I noticed that the text only was visible and not the logo itself (see attachment not the snapshots in this message).

    Here is the look I would like on several forms with the new logo to the left of the title.  The new logo is what i am trying to insert in place of the logo with the shield.  This form and the others I wish to use were imported from Adobe Forms Central.  Many of them have formatting issues.  Please help asap.  Thank you!

    You can instead use the TEXT field (from Form Tools) to have a more flexible Header Text/Image on the form.

    I will create a demo form base on this. I will get back to you shortly.

    You used the wrong logo.

    You may also use the header field to insert image and align the text to the right by injecting the following CSS code to your form.

    .form-header {

    text-align: right;


    For instance, the header field will be displayed as shown below :
