Remove sample logo image from the form header

I would also like to remive the SAMPLE LOGO image that displays at the top of the page. I cannot see it on the Form Builder when I try to edit.

You may remove the Sample Logo image from your JotForm from the Form Designer. 

You may click on the field and then click on Remove under Form Cover.

Now, save your JotForm.
