We have quite a few of your forms on our website. They have been working for months but have suddenly stopped. We are not getting notification emails when someone fills out a form. However I can send test emails for the forms when I'm logged in and they come thru but when people submit forms via our website we are not alerted. We haven't changed email settings.

I checked the email address related to your form and I can see the notifications are being sent, this is the email address : firstname.lastname@example.org.
Now, I see you are using your email address to send the email notifications, I would suggest you to follow this guide in order to do it properly : http://www.jotform.com/help/238-How-to-Add-a-Custom-Sender-Address-to-an-Email-Alert
