form out of thin air

Hi,

I checked your account and it was suspended for having forms that ask for bank account usernames and passwords.  You also have another form that asks for Facebook information.  All of which is against our terms of use.  Since all of your forms appear to ask for information that is against our terms of use, your account will remain closed.

If you need to create new forms, please do not ask for any usernames, passwords, account information or social media information.
