Link to form isn't working

    Hi there, 

    I have a form labeled Summer Art Camp. When I add the url to my website it pulls up a form that's different from what my summer camp template should be.  Any help would be greatly appreciated.

    I have checked the only form on your account entitled "Summer Art Camp Registration Form(http://www.jotform.com/40775888259171) and compared it to the form on your website and it is the same form.
