I am having the same problem.
No matter what, it gets blocked by uBlock.

You can whitelist our domains to your uBlock software instead. I tried adding uBlock in my Chrome browser and this is how it looks like in my end.
If I view your form's direct link, the request to block is lower or none.
If you have a lot of scripts loaded on the website, then it is possible that uBlock is also preventing them to load. Could you try embedding the form using the iFrame embed code instead? Here's a guide on how to get the iframe: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

I appreciate this enables me to see it, but this obviously is not a solution to my page visitors.

I tried to view the direct link as well and then it works.

I tried and the same result, blocked with uBlock.
I am just using a popular wordpress template, and uBlock does not block anything else on the site.
In your first picture, did you whitelist the domain before you took that screenshot?

I did not whitelist any of our domains, I just installed and visited your website in my end.

I do however see that uBlock seems to block Google Analytics scripts. Here's a screenshot of it:
I have limited knowledge on the uBlock so I am not sure how it should actually work. You can contact the developer/s here: https://www.ublock.org/. And report if it should block traffic or scripts related to anlaytics like Google. The plugin might be doing a false positive analysis on the script and blocking safe links instead.

Does Jotform have Google Analytic scripts on their forms?
It is strange why my uBlock block the site, but yours does not. That makes no sense.
I tried it with a couple of computers here at the office with fresh installs of uBlock, and uBlock blocks every one of them.

May I know how your uBlock is setup? I went ahead an updated the 3rd party filters (as I see that its the one filtered in your log).
I then tried it again and was able to see how uBlock is blocking the form in your website now:
I am not quite sure about the Google analytics script, but I believe it is related to our own "Form Analytics", our developers might be using Google Analytics for this.
As mentioned, this is quite out of our reach as this filter list is made by uBlock itself. It would be best to contact them for this false positive result.
However, I will forward this problem to our developers to have it investigated and see if they can directly contact uBlock or make some adjustments in the backend to lessen the probability of being added in the block list. We will update you on this thread as soon as we hear any news.

The filter which blocks `jotform.com` is part of EasyPrivacy, which is 3rd-party to uBlock. This means any blocker using EasyPrivacy will have the issue. The issue needs to be brought to the maintainers of EasyPrivacy: https://forums.lanik.us/viewforum.php?f=64

Unfortunately, as mentioned by gorhill, the list for the 3rd party filter is from EasyPrivacy. I have already informed our upper management about this. But I see that users are already reporting this to EasyPrivacy forum, here are the links:

However, it seems like removal of it in the block list will still depend on the authors. We're not quite familiar with EasyPrivacy but we'll check their forum and see if we can create an account and also report the issue. It would be best that you also contact them.

I have just updated my 3rd party filter list, it seems like the author/s of the EasyPrivacy have already remove JotForm from the block list. Please do update you list and see if it is still being blocked. I also clicked "Purge All Cache" before updating. Let us know if that works.

I have purged cashes both in uBlock and my browser. It still comes up blocked.

May I know if you have clicked the "Update Now" button?
I'm not sure how the "Purge all caches" does with the extension, I was thinking it is similar to clear cache. Mine still works after updating the 3rd-party filters.
I also see that one of the EasyList author already have removed JotForm in the block list. Here's the changed log they made: https://hg.adblockplus.org/easylist/rev/028b5a11cbe9
