Conditional Email Alerts No Working!! Second Time This Week

    Our conditional email alerts are not working when someone submits a form.  This is the second time this week that this has occurred.  You shared earlier in the week that you were re-designing the conditions template on your side.  It's down again.  I need it resolved ASAP.  Please advise when fixed!

     

    See form and data that have been submitted today that don't work!!
