I am not receiving emails for the below form. I have not received the last 2, then I just did a test myself and no email. This form has been sending emails no problem for ages.

Have you checked spam folder? Upon reviewing your notification, I can see the Sender Name is missing:
Sender Name :Make sure you assign it to a field from your form where the user enters her name (usually the"Name" field -or similar)
In addition to this, you may white-list our domain names, if the issue continues, as we use Amazon SES method for "email@example.com" sender.
