Envio de Formulários - Submission forms

Após digitar enviar formulário, quando clico em enviar, aparece a respostas de agradecemos o envio, mas a página de formulário fica parada nessa mensagem e não permite o envio de novo formulário, só se sair do site (app) e voltar. O que fazer?

After entering submit form, when I click submit, the answer appears to please send, but the form page get stuck in that message and does not allow sending new form, only leaving the site (app) and return. What to do?

I was able to submit your form without any issues so it's probably because of a conflict in the page, especially if you are embedding the form using our default method. Please try re-embedding it using our iframe method: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.
