I believe that the service that your ofhttp://form.myjotform.com/60165561814555ering would be cool, if we live in a perfect world, and there was no such thing as phishing scams. I got an email from one of your clients, and I truly believe that he is a scam artist. He is asking people to sign up to be a mystery shopper. If he were legit, his signup form would have been on his-own web addres.
The address for the form that he's phishing from is http://form.myjotform.com/60165561814555
He needs to be stopped, and brought to justice. Your reputatiion depends on it!
Phishing is when you try to obtain sensitive data from someone by scamming them about what they are doing. For instance if Russ Wilson builds a form called Bank of America Credit Card login where Russ Wilson tries to obtain data that would be sensitive and secret such as logins or passwords.
This form you present is someone asking you to apply for an opportunity to mystery shop by providing just sensible demographic information that you can easily refuse to provide.
If the email claimed to be from your bank or credit card or mortgage company asking you to update your password by providing your current one so that you are not locked out. That is phishing.
BTW- been a jotform user for 7 years as a serial entrepreneur. They are the fastest at shutting down actual scams just so you know and they make it nearly impossible for legitimate business owners to obtain information for legitimate and fully legal business means such as charging a credit card after the form is submitted without using their processors. It is against their rules.
Just so you know I aint blowing smoke here's the actual definition of phishing:

Thank you for reporting this to us. As mentioned by jasonsvctech, phishing is an act to where a person or group tries to masquerade itself as legal entity to steal sensitive information.
Forms like you have mentioned are somewhat common when people want to get general information from you, this is most likely to be used in subscribing you to newsletter OR possibly for spamming purposes.
I see that the account holder of that form is already suspended, I am not quite sure about the details of the suspension but it is most likely to be related to spam emails containing that form. You can check our "Terms" page to learn more about our service.
