My forms are disabled. How to fix this issue?

    I am having a problem with my last two forms which were not found and deemed disabled:

    Could you please assist in fixing the problem?

    Hello Bjarne,

    Upon checking your account I found that it's suspended by our anti-phishing system. As your form "Website Technical Information" has fields like "Username & Password" which makes it suspected of being used for phishing.

    Authentication form can't be used in Jotform and creating such forms are against the Terms of Use. I have deleted the password fields from your form and activated your account again. Do try to sign in to your account and get back to us if you have any trouble.

    Kindly refrain from creating form with email/password, login/password or credit card details.
