    Hi,

     

    You can insert a Text field on your form so you can add comments in the questionnaire.

    1. On your form builder, select Form Tools then click on Text

    2. Once a Text field has been added, click the Edit HTML button

    3. Type your message then click Complete once you are done editing

    Or if you want a comment to appear when a user hovers on a field, just add a Hover Text to that specific field.

    Click on the field > Click on Hover Text > Insert comment > Click OK

    Thank you for the quick reply.... Is it possible to show me about to comment if for example I tick yes in the box and if so, i could like to comment on that particular question

    If you would like to show a specific static text or comment based on a user input (like an answer to a check box), then that is possible by using conditional logic. You can learn more about it in this guide: http://www.jotform.com/help/57-Smart-Forms-Using-Conditional-Logic 

    Here's a sample form: https://form.jotform.com/60173288098968. Here's how I did mine:

    1. First, I added a radio button field so that user can only have 1 item to be selected. I'll insert a "Text" field for the comment or description.

    2. In your "Conditions" wizard, choose "SHOW/HIDE FIELD".

    3. Here's how my condition works, when "Show comment?" radio button is equal to "Yes" then SHOW the static text related to comments or descriptions.

    Hello Charlie,

     

    I'm still having troubles inserting the comment on a user input. It was successfully done in one of the demo's that I didn't before doing it on the real form.... however I'm not successful in implementing it into my real form.

    Hi,

     

    Are you referring to the form Introductory Questionnaire Sheet? I cloned the form and checked the conditions. It looks like this field successfully displays an add comment field when the user ticks Yes.

    Hi,

    In this particular form I have added in a comment into that field, as I couldn't get a comment that was invisible. I have tried manually without the added field however, when i wanted to preview my form the question doesn't seem to appear. What should i do?

    Hi,

     

    Your form seems to have errors in the conditions which might be why the conditions are not working as it should. Please remove the errors from the conditions and try adding the condition again.

    You also need to hide the fields first so that when a user clicks on Yes, the condition would then make the comment field visible.
