Is there a form that I can use to get someone's email address in return for a free product from our company?

Example: "Please type in your email here Mrs. Johnson, and in return for your email you can have this free product." Then the client's email is sent directly to our company email account so we can add it to our data base.

It seems to be a simple form with just Name and Email can be sufficient so that you may create an autoresponder to send the email to the form filling person. You may take a look at the following templates that might match.
