Why is my form displaying that it has been disabled?

    Hello there, 

    I have problem with my forms,

    same as above "FORM DISABLED" this form has been disabled .

    can you help me please im not phising this is my personal accounts for clients all my forms has been disable.. i dont know how i can explain this to my clients PLEASE !!! i need your assistance please ASAP ..
