Image attachments to submissions and uploaded graphics aren't loading

Images that have been submitted through forms and graphics we've uploaded to use in our forms aren't loading.  We can't view the attachments VIA email or through jotform and they are showing as a '?' when forms are accessed.  What can be done, or needs to be done, to fix this issue.
