Formatting completely wrong on pusblished form, yet design is correct

Hi there,
I have been using JotForm for about a year now, and have really enjoyed it, but just this week when I tried to make a new form, the form doesn’t show up the way it’s been designed… the form is found at https://form.jotform.com/60150888276967

The problem is that it throws the formatting out completely on the form and adds bullets.

How can this be fixed?
