My survey isn't previewing everything I have put on it.

I have created a basic form, it is displaying headline, name, email and phone number requests. But not displaying an additional piece of text and an additional text answer question I have put on it.

It seems the other changes that you did in guests account. It means, your session expired when you edited the form, so that the changes were published from a guest (cookies based) account.

However, don't worry about that, you can revert your form back to an older version.
