How to view all of the colors/graphics on a form

I created a survey with a background color and a heading title, but when I view the form, it doesn't have either.  It is just questions and answers in black on a white background with no heading.  

I checked your form and it seems to show the color and the heading correctly. Here's a screenshot of how I see it:

Could you please provide us more information on this? A screenshot of what you are seeing would help us a lot. Please also do try using a different browser and let us know if that gives you a different result.
