I think you should start by registering a for a free JotForm account.
As you have created that form under a 'guest' account, it may prove difficult, from an admin point of view, helping you recoonfigure your email notifications.
Just register for a Free account here -> http://www.jotform.com/page.php?p=signup using the same email address you used to make this post and your existing form will be automatically assoicated with your new account.
Once, that is done, let us know and we will take a look at this email problem for you.
