I'm sorry to hear about your issue seeing the last name.
Can you provide more information on where you're not seeing it? I checked your forms online, your submissions, and the notification format, and the notification emails that were sent. I see it in all those places.
Please let us know where you're missing it. There was a server hiccup over the weekend, so perhaps the issue resolved itself on its own.
