Form takes a long time to load or does not load at all

There is a form on one of our pages that has been causing problems with page loading for a week or two now, and today it is not loading at all.

I took a look at your form and it seems to load properly for me.  Looking at the same on your website however, I get 524 error - origin timeout.

As this suggested issue to be on our side, I have cleared your forms cache and it seems to be working properly now.
