Formularz nie działa

Pomimo zaznaczenia wszystkich pól formularz nie działa - nie da się wysłać zamówienia.

Wyskakuje informacja, że są błędy w formularzu.

Proszę o pomoc.

My apologies, there are so many fields and conditions in your form that I am having trouble finding the cause of the error.  Even if I disable the requirements on your fields, I am still unable to submit the form.  Was this working before?  Was there something added to the form at some point and it stopped working after?  Anything else you can let us know would be greatly appreciated.

everything was great until yesterday evening, the form just stop working.
It has been working for a few months very well. We haven't changed anything.
I have a file with something like "source" from man,who did this form for
us. Maybe it will help somehow?
Sorry for my english, but maybe it's more understandable than polish?

Your English is just fine.  I figured out which fields are causing the issue:

It is all 4 of the "Ile dni diety?" fields. Removing the requirement to fill in those fields did not help.  However, when I remove those fields from the form, I am able to submit the form without issue. 

Here is my test form with the last three "Ile dni diety?" fields removed:

I am still unsure what specifically about those fields is causing the trouble, I will need to do further testing.  If you wouldn't mind checking the conditions related to those fields to ensure they are correct, we will be happy to have another look.

else :( It's too difficult for my in english, so I try to write it in

Pole "Ile dni diety" określa czas dostawy oraz cenę końcowbr/>.
Cena zmienia się, w zależności od ilości dni dostaw oraz ilości posiłków.

Obecnie w tym formularzu https://form.jotform.com/60196873621965 źle jest
liczona cena.
Jeśli zaznaczymy pole "Z zestawem testowym" podaje błędnbr/> cenę, powinna ona
być taka, jak liczy formularz na naszej stronie:
Myślę, że on źle liczy teraz ten "Zestaw testowy" - to jest jeden dzień,
który powinien być w cenie - 50%.
Np.: Z zestawem testowym, 4 dni diety, Fit 1700 kcal, 5 posiłków
Cena: 66 - 50% = 33,00 (Zestaw testowy, czyli jeden dzień) + 3 x 66,00
= *231,00
zł*
Nie umiem sprawdzić, gdzie jest błbr/>d, bo nie wiem nawet gdzie to jest
zdefiniowane w formularzu :(
Będę bardzo wdzięczna za pomoc - nie mogę znaleźć nikogo, kto potrafiłby
obsługiwać ten formularz. Nie mam już kontaktu z osobbr/>, która go
projektowała, a to bardzo ważne dla nas narzędzie pracy....

Z tego, co rozumiem:
Każde pole "ile dni diety" było przyporzbr/>dkowane do innego zamówienia.
Przez nasz formularz można złożyć zamówienie:
1. Dla jednej osoby
2. Dla dwóch osób (drugi zestaw dla tego samego adresu)
3. Dla trzech osób (trzeci zestaw dla tego samego adresu)
4.Dla czterech osób (czwarty zestaw dla tego samego adresu)
Za każdym razem podaje się Ile dni ma trwać zamówienie.
W zależności od ilości zamówień sbr/> naliczane rabaty:
1. Brak rabatu
2. Dla dwóch osób - 5% rabatu
3. Dla trzech osób - 7% rabatu
4.Dla czterech osób - 10% rabatu
Teraz aktywne jest pole "Ile dni diety" nr 3, ono powinno być podpięte pod
zamówienie nr 3, dlatego źle liczy cenę.
Nie umiem znaleźć, gdzie jest to zdefiniowane.

If you could have whoever created the form for you, try disabling all of the conditions related to the fields I mentioned to see if that will allow the form to submit properly:

Unfortunately, I do not have enough time to sort through each one.
