Cannot use Application Form due to Security problem

    hi

     

    I sent someone the following online registration form and they replied

     

    I am afraid my health board says your website/online application form has a security problem so won’t allow me to use it.

     

    Do you know what may be the cause of this?

    I assume you provided this URL https://form.jotformeu.com/cmhabc/dental2.

    The URL was already using secure URL (https), so it is not clear what exactly was the security problem your end-user meant.

    It will help us determine the possible cause if we can get  more details about it.

    I also test submitted your form, and it did successfully and I see the thank you page. Please refer to video below

    Please let them retry also. Identify more details like which browser they used, or is there any specific error message they see when they try using it...
