Certain email notifications are not working. Can you please help me figure out why?

I'm having a major issue, too--it seems like the "reply-to" email field got switched with the "your email" field. There is no drop-down on "your email" where there should be so that I can select the email address field from my form. Instead, that drop-down is over in the "reply-to" email field.
Please disregard my response--I didn't realize there are two options when creating an email notification, and I chose the wrong one!
It looks like the email address connected to your account was in our bounce list, which is why the email notifications were not being sent to you. I have now removed your email address from the bounce list so you should be able to receive notification emails properly.
