my forms are not show or disabled

im using avanquest webesasy pro.

when i integrating the form is dosn't show anymore or shows this form is disabled, Why?

it was working yesterday but not today 

I've reactivated the forms and your accounts, but please do not create unlabeled our untitled forms as all this becomes suspicious for our system and it automatically triggers a script that suspend the account.
