Between May and August 2015 we published a questionnaire via JotForm and we had very good results, but a problem occurred during this period left us very concerned and unsafe to continue using your application. I wonder if we have assurances that the problem occurred not happen again, if it occurs, it is solved properly and quickly.
I beliave that you can find more informations about the problem in the application log.
