I just created and saved a new form; however, it will not work as it says the form is disabled. Why isn't it working?

    I made sure it was saved.  Tried Previewing it and it gives me an error sign that says this form is disabled.  I tried to publish it and the link doesn't work.  What step am I missing?

    Your account and forms were suspended by the Phishing filter due to your jotform http://www.jotform.us/form/60205532735146 that captures CC accounts.

    This is a violation on the JotForm Terms of Use.
