How to clone a form from an existing form ?

    I am new to jotForm. I am trying to turn our 2015 Registration form into a 2016 Registration form. I loaded a new image and tried to edit the prices and text. 

    Now, the image does not show up when it is supposed to be on the left side of the description. 

    And,

    I can't seem to find out how to edit some of the prices. The price for children on Friday night should be changed from $10 to $15 and I can't seem to find out how to make that edit.

    And,

    the title or heading is supposed to be 2016 Registration Form but when I turn this on to test it, the title appears as 2015 Registration Form

    Can you please help me as I've spent lots of hours at this already.

    And I was unable to find the cloned form you are talking about.

    Since, you are trying to clone a form from your account, I would suggest you to follow this guide : http://www.jotform.com/help/27-How-to-clone-an-existing-form-from-your-account 

    I cloned that form and  I was able to make all the changes you are trying to do.

    If you are trying to add an image and text at the same time, I would suggest you to use the Heading option :
