Form Calculation Widget : How to properly use Form Calculation widget to get a total amount ?

Hi there

I need to have about 9 different copies of this form (with different cost on each one).

So when I clone my first one,  the new forms will not total the cost.

What am I doing wrong?

Also I want these forms setup so that if someone checks an item or inputs the plan number that it will NOT complete the transaction WITHOUT hitting the "Submit" button. The only way to complete the transaction is with the Submit button.

I checked your form and I can see that the calculation is not configured properly, please, remove the + that is displayed or duplicated in the Form Calculation widget :

I did it and worked on my end, in regards to disable the submit button if the Plan Number field is not filled, you can easily achieve it by using conditions, your condition should look like this :
