How to fill an area for a notifier based on a response in a form?

I have a list of addresses I wish to put into a notifier email when someone picks a particular location.  So, when they pick location 1, I want to place location 1's full address (formatted correctly) into the notifier email.  I have a list of about 12 addresses that I'd need to put into a list somewhere and then access based on conditions. 

I tried doing this via a "calculate field" but the address doesn't come out correctly on the other side.

Help!

A possible workaround can be the following:

1) Create 12 notifications, in which you include an address on each notification.

12 notifiers isn't really an option.  The forms get turned into leads, and sometimes there are specific instructions and addresses that need to be put in the notifier email based on the input of the form.  I'm surprised there isn't a way to just "insert text here" (with a {texthere} or something) inside the notifier email based on a condition. 

Any other suggestions?  Where can I make a feature request if it doesn't exist?  I can't imagine that this doesn't come up frequently.
