My form is just not submitting and it's not telling me where the error or errors are. I had my wife try on her computer and I tried Firefox (I use Chrome) - no avail. same error.Page URL:

I'm able to replicate the issue at my end on a cloned version of your JotForm. It seems that one of the widgets are causing the issue on the form. I'm investigating the issue and get back to you once I have some helpful information on this.

I replicated the form and removed those two checkboxes. Works now. May want to fix those widgets though. Also, can u fix the sms verification widget... when the person requests a call instead of sms it says error. It's been like that for a few months, just stopped out of the blue one day.
