Seems like your account was suspended automatically because you have a form which was made on Arabic language.
Why is this? We have a tool called Phishing Detector which allows us to automatically suspend thousands of forms created everyday for phishing activities such as stealing credit card information, social security numbers, login credentials, child pornography, etc. However this tool is not updated to scan Arabic, French and other similar languages, therefore most of the forms made in these languages are automatically suspended.
To avoid further suspensions we recommend you to build the form in English and then use the Language Translator tool to set it into Arabic, French or any other language that might be suspended automatically.
Simply follow this guide to make your forms multilingual: How to make your forms multilingual
I checked your form and it doesn't seem to violate our terms. I have now reactivated your account so you are able to edit your form and make it multilingual.

I don't understand this. will this happen all the time because of your automated services???
I can't work like this. It's not professional. I'm waiting for hundreds of submissions to a prestigious event. this looks really bad.
I need your guarantee it won't happen again.
otherwise i'd have to use a different form service

Yes, your account will be suspended all the time by the phishing detector tool if your form is in this language.
The solution for this is to create your form in English first just like what Christian said. After that, use the Language tool to set the another language as the default.
Please check this demo form: https://form.jotform.com/60273556321956. I created that form in English first. After that, I use the language tool and make Hebrew as the default.

By the way, I have now re-activate your account. Please update or change your form as soon as possible to avoid getting suspended again.
