Form is messed up on Firefox

    This is what I see on Firefox. Mobile and Safari are fine.

    Well, it somehow resolved itself. It is OK now.

    I presume you are referring to the width of the form fields? Could you please try re-embedding using the iFrame embed code instead? 

     

    If you are referring to the text you are seeing on the input fields that looks cryptic, this is because you have enabled the "Encrypted Feature" of the form, please do make sure to read this guide about it: http://www.jotform.com/help/344-What-are-Encrypted-Forms-and-how-to-use-them-as-expert
