Only part of my form shows up in transmittal

Form Scholarship_Application(2016) or Scholarship_Application_Test on http://www.aauwvalparaiso.org/Scholarship_Application... looks normal both in FrontPage and on the web, but when I send it to recipients only part of the form shows up

The URL you've provided seems incomplete. We are redirected to a 404 page.

Anyway, you're right and your form is working just fine. there's no cut off when I check it here.

May we know what URL you've shared with your users? Is it your page URL or the form URL? 

If you can provide a screenshot that shows the problem, we'd appreciate it.
