Hi...the new form created below is not working. I "cloned" an in use form to create this one and subsequently deleted some fields. On submission it shows these fields as still being required as you can see from the attached screenshot.
I'd be grateful for your help.

I've tried to submit your JotForm and I was able to submit the form without any issue.
