Thank you for getting back to me so quickly. You have the correct form. You also have the correct entry (ie Miguel Zamora). I have the email which you included in the your email.
The situation is this. I download the submissions (with 30+ elements in each submission) regularly. The data (in excel format) feed into a database we used to track enrolment. For some unknown reason, Miguel Zamora's submission wasn't on this excel download.
I noticed this between the database showed one figure we collected while our PayPal account showed something different. I wrote because I had no idea of what happened. I am the only one that accesses JotForm. I know I didn't delete any submissions. As a matter of fact, I still have submissions from the past years. Please look into this for me.
Onto a separate issue. I mentioned that I received duplicate email from PayPal. I am not sure if these were from you or PayPal. Nevertheless, let me give you some examples.
Jan 25, 12:01 am. I received two identical PayPal emails for Joey Schweyer's registration.
Jan 26, 2:25 pm. I received two identical PayPal emails for Jamie Cushing's registration.

Now looking into your submissions I was able to find the entry you had mentioned. Its submission ID is: 329635669071695443
You can use ID to search for it in Submission Panel and next to that, I have marked it with a flag for you so that you can find it easier. At this time it is on 2nd page of your submissions.
