Why has my form reverted to an older version without all of my updates?

    My forum has reverted to an older version without all of my updates. How did this happen? This is a very big problem for me. It was saved. The first few people filled it out fine. than is just changed over night. I have shut the form down as of now. Please help. Can you find the form people were filling out as of 10:30PM last night?

    I have check form revision history of form you provided and I saw that you made a lot of changes today on your form. 

    Please use Form Revision History option to revert your form back to an older version. Inside of his guide you can find out how to view form revision history:

    Why do these not match? First two are what the current version say. The third screen shot is what I see and others when they fill it out. How do I get them to match. I think there is a problem on your side.

    Considering that you form is disable I have cloned it to my account. Here is my cloned form:

    Does your form looks like this as well? When I checked form revision history on your account i saw that below image you have drop down field. Is that the field that is missing from your form? Did you tried to add it again? 

    You can also try to revert your form to some other version and then to revert it back to latest version.
