My form has disappeared

Yesterday I created one form called YCC-2016 but it's disappeared today from list. Can you please check it what is going on and recover back that form

Unfortunately, I can't find it using your IP Address. You have a form in your trash folder, but I don't think that is the missing one. Do you still have the link to that form or embed codes? Let us know so I can search further.
