I've fixed it for you. You have to move the field to the new line. Please review this guide for field positioning: http://www.jotform.com/help/90-Form-Field-Positioning.

I knew that you had to shrink the two fields, but that does not work see the form
And sadly I had left the form open and I think it overwrote what you did.
I have shrunk both fields in the current version and as you will see the Name field is justified to the right and shows that way in the preview.
I close the form this time !!

I forgot about the "New Line" on the dropdown, used that, now all good to go.
