Forms are not working!

I am trying to embed forms onto my website and they are not working in any format, HTML, source, or normal script way!

All the other ways just plain don't show up. Although I see the code in my webpage and checked for js conflicts. BUT when I put in the "iframe" view I get a box and the error shown in this thread: http://www.jotform.com/answers/751131-Form-engine-failed-8j1h17f1 "Form Engine Failed #8j1h17f1"

PLEASE HELP

See the attached link. The form is SUPPOSED to show where it says "Contact Form" within that box. Thank you
