Not sure if I understand your concern correctly.
I checked both forms:

Both are in your account. However, I noticed that the 2nd form is blank. I checked your history and it was created last October 26, 2015. There were also no changes made on it, it seems to be blank the day it was created. You can check your form's revision history by following this guide: http://www.jotform.com/help/294-How-to-View-Form-Revision-History to see the different state of it.
Is the form transferred or moved from a different account? We'll wait for your response.

All is good. I included the second link by mistake, and I'm happy to report that the first line (https://form.jotform.com/52665036929969) is now working again.
