You will have to create a custom fields in Infusionsoft that will match the custom fields in the form.
Please check this articles from Infusionsoft regarding custom fields mapping
1. http://ug.infusionsoft.com/article/AA-00224/0/Create-custom-fields.html
2. http://ug.infusionsoft.com/article/AA-00233/0/Choosing-custom-field-types.html
Hope this help. Let us know how it goes

Ok, I restructured the fields for that.
I was sent this video for the integrations and how to do the file upload to match a custom field inInfusionsoft.
http://help.infusionsoft.com/mastermind/jotfrom-upload-files
Apparently its a different version of Integration on Jotform?
When I do the file upload, jot doesn't send the full url to my custom field like in the video. (see video minute 21:00)
Any thoughts?

I have moved your concern to a new thread because we believe that we can better assist you if you will have a separate thread for every question that you have to avoid confusion with the answers. You can follow the new thread by clicking this link: Infusionsoft-Integration: Unable-to-get-the-link-to-the-file-from-File-Upload-passed-to-Infusionsoft-Contact-custom-field
