I have posted this question previously, but I am trying to make it available by February 1st, and I would appreciate it if someone would have time to look at what I am doing wrong.
My form looks OK on my computer and looks OK on the web, but when I receive the submitted form via email I only get the first 15 - 20 lines.

I can see that the problem is with the email notifications. It is showing all the answers but not all the questions. Please follow the steps below to edit the notification email content.
1. Click the "Emails" button in the toolbar.
2. Click the "Edit" button in the selected notification.
3. You'll see the "Compose Email" wizard where you can edit and fully customize the email that you'll receive. You can add or edit the questions, and add the user's answers to it.
You can see that there is no "question" from {input22} up to {input36}, they're empty or blank. The reason is that on your form, you didn't use the "label" as the question. You use the text tool, which is considered as another field.
I would recommend you using the "Label" as the question. Click the "gear or options" icon and then select "Edit Properties".
Here you can put the question which is the label of the field.
After doing that, just delete the existing notification and create a new one so that the changes you made will be applied automatically.
