My account name is Jennifergrainger. The form I am referring to is the 15 point checklist.
The participant answers true or false to each question using a check box. At the end of the checklist will be a "quiz results" letting them know where they stand depending on the number of TRUE answers they checked. It would be awkward for them to have to go back through the 15 pages to count their own number of TRUE answers. Being able to tell them at the end of the checklist how many they had would be useful. Can this be done?
ALSO, can they print out their completed checklist when then are finished? (or at anytime, if they'd prefer to do the exercise off line)?

Once you have created a form, you can make this into a quiz using our QUIZ APP. This will enable you to tally the result and display the result at the end.
Turn your form into a Quiz or Exam
You may also print a form by adding the PRINT BUTTON

If you had a change to clone the Randy's form, you would see that he created a 2 page form. The first page contains all the true/false questions while the second has the calculation fields to display the result.
Each question had a calculated value 1 for true and 0 for false. The calculation fields just add this fields together to display the result.

I have inspected calculation that is set inside of your demo form and I think that this is the easiest way to calculate number of True and False options.

Thank you for taking the time a few weeks ago to respond to my previous
question about getting tabulated results on a Jotform. This led me to
the Quiz App. I have been struggling with this project for way too long
so I am reaching out to you again in hopes you can steer me in the right
direction.
Here is what I want to accomplish:
1. I have created a 15 point checklist as a lead magnet for my website
and converted it to a quiz.
2. I used your suggestion of using radio buttons so they would get the
results of both the true and false answers. So far so good.
What I need help with.
1. I want the 2nd page of the quiz that has the test results to also
have the Scoring Key. As it is now it just shows the questions and
whether their answers are right or wrong. How do I add the Scoring Key
to the results page (without it also being at the end of the 1st page)?
2. I want the email they get to be a copy of the 2nd page so they have a
record of the questions with their score and scoring key.
3. How do I also get that same email sent to me?

Jeniffer, in the demo form set up by Randy, the form is not yet submitted when you see the results. In order to get the form submitted, so that notification emails can be sent and a Thank You message shown to the user, the user would have to further click on the Submit button on that second page (result page).
In order to achieve on your own form what Randy set up in the demo, I will walk you through the steps manually.
1. We need to assign calculation values to the answers of your fields - assign 1 for "True" and 0 for "False":
We would do this on all 15 questions.
2. We would add a Form Calculation widget to your form to calculate the combined value of all selected answers on your form:
In the calculation widget, we simply use the Add Field button to add each calculation field to your form, and then add a plus sign before adding each new field to the calculation:
Now we can use this as a result field both on your thank you page, and in your emails.
3. In order to hide the calculation field from your form, simply click on its gear-shaped icon and select Hide Field:
4. To have the calculation for failed answers as well, add another calculation field to your form and set it to calculate "15 - total score calculation":
5. Hide this field from end users in the same way as step 3.
6. This allows you to have your own Thank You message, where you can let the user know how many correct/incorrect answers they have had:

7. I would also highly recommend marking your questions as required, so that the form cannot be submitted unless all 15 questions have been answered. You can do that by clicking on a field, and then on its Required button at the top:
I am not sure what you meant by "Scoring Key", so could you please describe it in a little more detail?
To have the email with results or any custom message sent to your user, you can set up an autoresponder, for which you will have to have an email field on your form. To have an email sent to your address as well, you will also need to set up an email notification.

Thank you for your help. I have had help from several different sources
so I don't know how I got the link to the custom thank page associated

but the custom thank you page has this URL

want to add the Scoring Key which tells the quiz taker to match the

If you are using the quiz form app, you can edit the thank you page on the quiz app itself.
1. Go to the quiz form app and add your quiz form.
2. On the setup page, click the two checkboxes seen below to open the custom result editor
3. Input your Scoring Key on the editor
4. Save quiz when done editing
Your thank you page should now have the Scoring Key just like in this sample form I have created.

What I can't do on the standard Thank you page is show all the questions
with their answers marked as correct or uncorrect. This is VITAL
because the whole point of this quiz is to show the quiz taker how much
they don't know. If they don't get the results of their WRONG answers
associated with the questions they got wrong, it is a wasted lead
magnet. It does no good to indicate you have this number of answers
correct and this number incorrect if it is not related to the actual
questions they got wrong.

you will see the
second page that comes up is a custom page created by somebody who was
trying to help me (so many, I don't know who it is). This page comes up
because on the Thank You page it is redirected to
http://quizform.jotform.io/thankyou.php. If I could access that I could
add the Score Key and I'd be done and able to get on with making a
living. For whatever reason if I just open the redirect URL in my
browser I get an error message so I don't know how it is that it is
associated with this jotform. I know someone knows how to do this
because there it is. I just need to add the score key to it. Maybe you
could look back at all the messages associated with this problem and
find the person who created the php.

Hello Jennifer,
I have looked at your form '15 Point Sales & Marketing Blind-Spot Checklist' it seems to redirect to the Thank You page you mentioned but without the error. I was able to fill the form and view the result with their associated key.
Maybe the error was just temporary.
If this is not the problem you were referring to please let us know we will be happy help you get this done.

competent writer who can clearly state a problem, but I am not getting
through on this one.
1. Yes, upon clicking the submit button the jotform goes to the right
thankyou page (which is a custom page created by someone else I don't
know who, so many people have been involved in this issue for DAYS.)
2. I need to add text to that page: the Scoring Key so the quiz taker
knows what their answers mean. But If I try to access the php file so I
can add the text by entering the URL into my browser

When the quiz taker clicks the Submit button the thank you page comes
up. That is NOT the problem. The problem is I don't know how to access
that file so I can add text to it so the score has meaning to the user.

Hello Jennifer.
1. Please note that the Thank You page you are referring to was not created by anyone in the support team. That thank you page is automatically created when you use the Quiz Form app. That app is where you set the correct answers in your form.
2. When you are using the quiz form app, you do not need to go to the Thank You page. This is why you are getting the error messagge. To edit the result of your quiz form, you will need to go to the quiz form app by following this link: http://quizform.jotform.io/
3. To get to the Thank You page of your quiz form, please follow this link: http://quizform.jotform.io/
Then you can simply follow the guide below to edit your quiz form Thank You page.

In that form you will see that when the user submits the form, they will be directed to a Thank You page where I have added the Scoring Key.
