In the email confirmation, I have entered the fields for name and amount paid. However, they are not showing up. Address, email, etc are showing up, but not the name and amount. See attached screenshot.
How can I fix this?

I checked your form and it looks like you have already updated your autoresponder to include the new field ID for your name field. When I submitted a clone of the form, the name field was indeed forwarded:
If you are still having trouble with this, let us know and we will be happy to have another look, as far as I can tell though, you should be all set.
