Address Field Not Displaying

    Hello,

     

    I have a form on my account https://www.jotform.com//?formID=60056964220956 that is not displaying the address fields that I inserted to the form. It shows on the builder, but not on the preview or the live version of the form.

    I cloned your form and was able to replicate the issue.

    It seems like there are errors in your conditions which is causing the Address Field to not be shown.

    Deleting these conditions should fix the issue.

    I was able to see the address field in the cloned form after I removed the conditions with errors.
