How do I change the settings to enable the customer to choose multiple options in a survey form question?

There are a few ways to enable your customer to choose multiple options. In the form editor, you can go to Survey Tools and select Matrix:
...and you can edit that to provide options for multiple questions.
Or, you can go to Form Tools and choose a Dropdown:
...which offers options.
Or, you could use Radio Buttons:
As I mentioned, you have many options. If you like, you may find lots of helpful information on building forms here.

That's a good advice Charles.
Additionally, I just would like to mention that you can also use the check boxes, you will find them under the form tools option, in the form builder:
You can also allow to select multiple options in a drop down:
And when using the matrix field, you can choose which type of field to use:
Hope this helps.
