your jotform app does not appear in the salesforce app exchange. There is nothing at salesforce.com about generating a token

For the SalesForce Integration to work properly you have to go into your SF Account and generate it.
That token is what helps to allow our integration to connect to your account in salesforce otherwise it won't work.
To obtain it go to salesforce and while logged in look at the top navigation bar then go to Your Name > Setup > Personal Setup > My Personal Information > Reset My Security Token.
